Folate and Alcoholism

چکیده
Alcohol has many effects on folate metabolism. Excess alcohol consumption is often associated with a poor (folate deficient) diet and malabsorption. Alcohol accelerates the induction of megaloblastic anemia in subjects on a low folate diet. It antagonizes the hematopoietic response to folate (75 yg/day) in folate-deficient alcoholics, but its effect can be overcome either by larger doses of folic acid or by cessation of alcohol ingestion. Alcohol also induces an acute fall in serum folate which is rapidly reversible on withdrawal of the alcohol and is not correlated with depletion of folate stores. This acute fall in serum folate is not due to an assay artefact and may be due to the interference by alcohol with the delivery of folic acid from stores to the peripheral circulation. While the exact mechanism of alcohol's effect on folate storage and release by the liver is still under investigation, alcohol may produce a reversible sequestration of folate in the hepatocyte (as intracellular polyglutamate) at the expense of its release into bile as the methyltetrahydrofolate monoglutamate for recirculation to tissues. The significance of alcohol-induced changes in folate binding and folate binding protein to the delivery of folate to tissue stores is unclear. Thus, the relationship of alcohol with folate deficiency and megaloblastic anemia is multifaceted.

Detection of folate deficiency in alcoholism using the peripheral blood lymphocyte deoxyuridine suppression test.
Twenty-seven alcoholic patients were studied for folate status. Twenty-two (81%) had neutrophil hypersegmentation and were detected as folate deficient by also having low serum folate, erythrocyte folate, lymphocyte folate and/or an abnormal peripheral blood lymphocyte deoxyuridine suppression test. Seventeen (63%) had an abnormal lymphocyte deoxyuridine suppression test and all 17 were correct...
متن کاملAlterations of homocysteine serum levels during alcohol withdrawal are influenced by folate and riboflavin: results from the German Investigation on Neurobiology in Alcoholism (GINA).
AIMS Various studies have shown that plasma homocysteine (HCY) serum levels are elevated in actively drinking alcohol-dependent patients a during alcohol withdrawal, while rapidly declining during abstinence. Hyperhomocysteinemia has been associated not only with blood alcohol concentration (BAC), but also with deficiency of different B-vitamins, particularly folate, pyridoxine and cobalamin. ...
متن کاملBiochemical and Molecular Mechanisms of Folate Transport in Rat Pancreas; Interference with Ethanol Ingestion
Folic acid is an essential nutrient that is required for one-carbon biosynthetic processes and for methylation of biomolecules. Deficiency of this micronutrient leads to disturbances in normal physiology of cell. Chronic alcoholism is well known to be associated with folate deficiency which is due, in part to folate malabsorption. The present study deals with the mechanistic insights of reduced...
متن کاملGenetic polymorphisms relevant for one-carbon metabolism show no effect on homocysteine plasma levels and DNA methylation in alcoholism.
The objective of this study was to investigate whether polymorphisms of genes that are involved in one-carbon metabolism (dihydrofolate reductase, methionine synthase reductase, methylenetetrahydrofolate reductase, reduced folate carrier 1 and transcobalamin II) influence DNA methylation in 106 patients with alcoholism. In the multivariate model no genotype showed significant effects on DNA met...
متن کاملHyperhomocysteinemia in chronic alcoholism: correlation with folate, vitamin B-12, and vitamin B-6 status.
Serum homocysteine concentrations have been shown to be a sensitive functional indicator of intracellular folate, vitamin B-12, and vitamin B-6 status. Chronic alcoholism is known to interfere with one-carbon metabolism, for which the above vitamins serve as coenzymes. In the present study, these vitamins were assessed in 32 chronic alcoholics and 31 healthy volunteers by measuring blood vitami...
متن کاملNeurological disease associated with folate deficiency.
In a general medical hospital population the neurological status of 24 patients with severe folate deficiency was compared with that of a control group of 21 patients with normal serum folate. A significant increase of organic brain syndrome and pyramidal tract damage was found in the folate-deficient group. These findings were independent of the degree of anaemia or the presence of alcoholism....
